You or your carers may be facing ongoing problems in everyday life, or an unexpected crisis of ill health or bereavement. The aim is to provide you with the information to make plans and changes and to have the maximum amount of choice about the direction and quality of your life, in the years when a certain amount of practical or emotional support may be required. The social worker will be able to help you take stock of your situation, identify your needs and work on improving the quality of life and thus your general wellbeing. They are also available to assist with general enquiries and may be able to direct you to other sources of help and advice on specific issues.

ACCIDENT
AND EMERGENCIES
The nearest casualty department open
24 hours is at Glan Clwyd Hospital, Bodelwyddan. This department should be used
only for emergencies, when there has been an accident or when a severe or acute
illness develops necessitating immediate treatment.

All members of the practice’s
Primary Health Care Team are very clear in their duty towards patients and the
practice. If you feel that confidentiality has been breached please contact the
practice manager. The practice places great emphasis on its duty of care to patients.
The practice is registered with the Information Commissioner.
CHILD PROTECTION
The practice's ultimate responsibility is always the safety and welfare of children. The practice reserves the right to disclose necessary information to Social Services if it is considered that a child may be at risk or in need.
CHAPERONES
There are occasions when patients may need to be examined by a doctor which may involve an intimate examination.
This practice is committed to putting patients at ease whenever possible. If you wish a chaperone to be present during your examination, please do not hesitate to ask the doctor for a chaperone.
It may not be possible for such a chaperone to be provided immediately; you may have to return for the examination to be carried out at a mutually convenient time.
Trust is important in the relationship between general practitioner and patient; we would, at all times, wish you to feel able to ask for a chaperone should you require it.
OVERSEAS VISITORS
Individuals who are not normally resident in the United Kingdom are NOT entitled to free primary health care. Overseas visitors, even if they are British citizens or from countries with a reciprocal healthcare agreement, will have to pay private consultation fees and full prescription costs for any medications prescribed. The latest rates and fees are obtained from reception.
